Taking your first wellness assessment

Your initial wellness assessment helps Kivly understand your current state and provide tailored recommendations for your journey.

What is the wellness assessment?

The wellness assessment is a comprehensive questionnaire that evaluates:

  • Physical health and activity level
  • Mental and emotional wellbeing
  • Sleep quality and patterns
  • Stress levels and triggers
  • Social connections
  • Life satisfaction and purpose

How long does it take?

  • Time required: 10-15 minutes
  • Questions: Approximately 40-50 questions
  • Format: Multiple choice and sliding scales
  • Can be paused: Save and return later if needed

Accessing the assessment

For new users:

You'll be prompted to complete the assessment immediately after creating your account.

To retake or update:

  1. Go to ProfileWellness Assessment
  2. Click Take New Assessment
  3. Complete all sections
  4. Review and submit
